Follow these steps if you ….
Forgot Your Username	Forgot Your Password
Click FORGOT USERNAME Complete Fields (Email & License Number, if applicable) Click SUBMIT (Once account has been located, your username will be emailed to you)	Click FORGOT PASSWORD Complete Fields (User name, Email & License Number, if applicable) Click FIND MY ACCOUNT (Once account has been located, A code will be emailed to you) YOU MUST ENTER CODE AND ANSWER THE TWO SECURITY QUESTIONS Click VERIFY (Once Verified...It will prompt you to create a new password)

If unsuccessful…nurses, residents, medical clerks, and funeral home staff should contact your designated EDRS Administrator.

Need to Register ? If you are a ......
Nurse, Resident, Medical Clerk or Funeral Home Staff	Contact your EDRS Administrator
Physician or APN	Click on Need to Register and then I am a Physician or APN.
New medical facility New funeral home	An administrator should click on Need to Register and then I am a Facility Administrator. Follow the instructions to complete, sign, and fax the form.
Registrar	The Local Registrar should contact the OVSR Outreach Manager for assistance.
