All Funeral Home Administrators and Medical Facility Administrators will be given their username and password when they attend a Training class. If you do not attend a class, you are required to fill out a form and submit it to NJ-DOH to obtain your username and password.

5. Scan and email to edrssupport@doh.nj.gov
Once the information is verified, you will be contacted through email with the login information.
