Ordering Copies

If a Funeral Home wishes to obtain Certified Copies of a decedent's Death Certificate, users may do so via the Order Copies link on the Actions menu. They provide details as to who to request the certified copies from and how many copies with and without the Cause of Death. They also indicate the delivery type: Pick-up, First Class, or Overnight.

 

Fees for orders are deducted automatically when the orders have been successfully issued by the designated registrar. Pending orders may be canceled at any time by clicking on their corresponding Cancel button since they are not debited until the copies are printed out by the registrar. Past orders are shown under the subsection called Previous Orders and the columns are as follows:

 

Request Date 

The date and time the order has been placed.

Issue Date

The date and time the order was processed and the fees deducted.

Short Form Qty. w/ Cause

The number of short-form copies requested with the Cause of Death information.

Short Form Qty. w/o Cause

The number of short-form copies requested without the Cause of Death information.

Long Form Qty. w/ Cause

The number of long-form copies requested with the Cause of Death information.

Long Form Qty. w/o Cause

The number of long-form copies requested without the Cause of Death information.

Method of Delivery

Whether the order will be picked up, sent first class or overnight.

Total Cost

The total amount charged for the order.

Registrar

The name of the registrar who processed the order and issued the certified copies.

 

 

To order certified copies:

 

1.Click on the corresponding case ID from the Welcome screen.

 

2.Click on the Order Copies action from the Actions menu.

3.Designate the registrar that will issue the certified copies.

4.Specify the number of long or short form copies with the Cause of Death and/or the number of long or short form copies without the Cause of Death. If the long form is ordered without the cause of death information, redaction surrounding the medical section of the certificate will occur. To preview with the certified copy will look like, simply click on the image icon next to the format you are interested in.

 

       Here is a description of what each type of copy entails:

       Short Form With Cause of Death is an 8.5" X 11" copy which is a summary of the death certificate.

       Short Form Without Cause of Death is an 8.5" X 11" copy which is a summary of the death certificate, excluding any cause of death information.

       Long Form With Cause of Death is an 8.5" X 14" copy which includes the entire death certificate.

       Long Form Without Cause of Death is an 8.5" X 14" copy which includes the entire death certificate, redacting any cause of death information.

5.Provide the method of delivery by selecting an option from the drop-down list.

6.Click on Calculate Costs to see the breakdown of the costs and your account balance to date.

7.Click on Order Copies to proceed with requesting the copies after you have reviewed the order details and costs.

8.[Cancelling an Order] You will notice that your order has now been placed and you may cancel it if you like by clicking on the Cancel button. (Fees are not deducted until the Local Registrar prints the certified copies.)
