To begin using New Jersey's Electronic Death Registration System (NJ-EDRS), every user must be registered to the system and utilize a personal username and password from New Jersey’s DHSS, the Department of Health and Senior Services.

Registering for the first time:

 

1.If you are a new user to NJ-EDRS, you will first need to register before you can access the system. If you belong to a facility that already uses EDRS, chances are the facility has an EDRS administrator who will be able to register you onto the system automatically; simply request him/her to do so. You will then receive an email with your login information. On the other hand, if no facility administrator exists, simply click on the Register as a new user link as displayed below.

2.From the menu that appears, select what kind of user you are:

 

Physician
Registered Nurse
Mortuary Practitioner
Local Registrar
Facility Administrator - to manage users within a facility
or Other

 

3.If you are a physician, you will have the capability of self-registering for immediate access to EDRS. All other users will be required to fill out an application.

Logging in:

 

1.If you already have a user name and password, simply click on the user name field first and type in your assigned user name. Note that there are no spaces allowed in user names, only alphabetical and numerical characters are allowed.

2.Move to the next field, the password field, and type in your password.

3.Click on the Login button to sign in.

If you have forgotten your password:

 

1.Click on the Forgot Password? link.

2.To retrieve your password, EDRS will require the User name. You may instead supply both of the following: License Number (i.e. Physician's license number, Funeral Director's license number, etc.) and Issuing State (applicable to out-of-state physicians). After the required fields are completed, click on the Submit button.

3.Shortly after you have submitted the form, EDRS will e-mail you your current password based on the email address supplied in your EDRS profile. If an email address was not supplied in your profile or is being shared by other EDRS users, you will be notified to contact Help Desk at EDRSSupport@doh.state.nj.us or 877-797-4796 for alternative methods of password retrieval.

If you have forgotten your user name:

 

1.Click on the Forgot user name? link.

2.To retrieve your user name, EDRS will require the email address.
